Compromise and Release Form

Effective February 10th, 2011, the Bureau is requiring all parties to use a new version of the Compromise and Release Agreement Form.  The new version of this form can be found at the Bureau’s website www.dli.state.pa.us.  This form and many other related forms, can be downloaded directly from their site.
